Community Healthcare of Texas offers in-home palliative care for those patients that are managing a terminal diagnosis and receiving ongoing medical treatment. The palliative care program allows in-home nurse and physician visits as well as telehealth access. Services include individualized goals of care, medication management, disease education, advance directives planning, and support and education for caregiver and family members.
For those patients that are ready for hospice care, Community Healthcare of Texas-Providence Hospice provides inpatient care, continuous care, routine home care and respite care. Services are offered wherever the patient calls home including residential home, nursing facility, hospital or specialized inpatient facility. Their team of physicians, nurses, social workers, spiritual care coordinators and aides are dedicated to providing support and exceptional care to those in need.
